General Terms and Conditions
By applying for a United Health Awareness Foundation scholarship, you agree to these terms:
Your UHAF membership must be validated and active.
For scholarships requiring a high school diploma or GED, you must be either a current college student or plan to enroll in college within the next 12 months.
You must be a U.S. citizen or permanent resident to be considered for UHAF scholarship awards.
You must provide your mailing address so that we can send you paperwork to complete if you are selected as a scholarship recipient.
To be awarded a scholarship, recipient must enroll at an accredited college or university located in the United States. Scholarships proceeds will be remitted to that school, not to you directly.
By applying for a scholarship, applicant grants UHAF permission to post their name on unitedhealthawareness.org or in other marketing communications in the event they are awarded the scholarship.
Scholarship recipients will be required to submit a current photo and submit a publicity release granting UHAF permission to use their photo on marketing communications.
UHAF reserves the right to change the scholarship programs at any time, including available scholarships, deadlines, requirements, award amounts or other program details.
Your information is safe with us. The UHAF Privacy Policy applies to all UHAF scholarships.
Scholarship winners will be announced on the unitedhealthawareness.org website and will be notified by the executive director via US postal services.
The application package must include TWO (2) letter of recommendation and an official transcript (with raised seal).
